Every formal risk lives here with band, treatment, residual position, and review dates. Entries are evidence-backed, not casual notes.
In the orchestrated model, many risks are **created when a Decision Pack is approved**, not typed from a blank form. The register remains the audit spine for fixes, consultations, and assurance.
Filter open, accepted, or monitoring views as you would a live register — not as your only daily inbox (that is the exception cockpit).
