Maturity review
Not “what's the risk today,” but “how good are our habits, generally?”
This is a step back from any single risk, to ask how developed and consistent the organisation's overall practices are — things like how well changes get consulted on, or how rigorously fixes get checked afterwards.
It's useful precisely because an organisation can have zero open risks today and still have weak, inconsistent habits that let a new one slip through unnoticed. This page asks about the habits, not just the current scoreboard.
Each practice area gets a score and an honest label: still basic, developing, or well established — with a named next step for anything that isn't yet established.
For example
“Effectiveness review” might be scored as foundational, with the suggested next step being to define clear before-and-after indicators for every closed fix.
